Founded in 2006, the Primary Care Collaborative is a not-for-profit organization comprised of a diverse range of members, including payers, providers, non-profits, and major corporations. Our mission is to enhance the efficiency and effectiveness of the health care system by promoting a strong emphasis on primary care and the patient-centered medical home model.
