HealthEZ, originally founded as Family Health Plan in 1982, was the pioneer of the first PPO in the nation. In 1983, the company integrated medical management services with its network, offering comprehensive solutions to various insurance companies, third-party administrators, and large employers, including notable clients like Northwest Airlines and the Fairview Nurses union plan. Now operating as HealthEZ and America’s PPO under The Araz Group, the company has grown to employ over 150 staff members, primarily based in Bloomington, MN. With four decades of experience as an independent network, managed care services provider, and third-party administrator, HealthEZ is well-equipped to deliver the services outlined in this proposal.
